MY HEALTH FOR LIFE - IS IT FOR YOU? My health for life is a free healthy lifestyle program designed to help people who are at high risk of chronic disease get their health back on track. It could be the ideal program for you! The My health for lifeprogram recognises that everyone is different and that joining a gym, or having special “diet” food delivered does not work for everyone. We also understand that having support is a major factor in staying healthy. We focus on teaching you the skills to help get you going, rather than just giving you information about what you should be doing better. Our communication is two-way, and we work with you in a partnership. If you have been diagnosed at risk of chronic disease, then our goal is to work with you to get your health back on track and lessen that risk.

WHAT'S INVOLVED My health for life offers six sessions of personalised coaching and support over six months. Your first session is a one-on-one, personalised appointment to help tailor the program to your needs. Session two, three, four, five and six are either delivered in small groups based in your local community, or they can be arranged via the phone at a time that suits you. The choice is yours. In session six we reflect on the progress you’ve made to date and how to keep on track, going forward. In between sessions, we offer a range of support, including information, tools, tips and ideas. And when you finish the program you will be able to access our online Maintenance Program which is only accessible to other participating members on a similar journey.
Sessions will cover:
Understanding your personal risk of chronic disease
Understanding conditions such as type 2 diabetes, heart disease and stroke
Understanding how lifestyle choices impact on health
Making change in manageable steps
Goal setting and action planning for a healthier lifestyle
Healthy eating and moving more
How factors such as stress and sleep impact on our health
Maintaining good health and healthy behaviours.

ELIGIBILITY My health for life has been specifically designed for people who are at high risk of chronic disease. You are eligible for the program if you are:
45 years and over and have been identified by a health professional as being at high risk of type 2 diabetes, heart disease and stroke; or
18 years and over and of Aboriginal or a Torres Strait Islander descent and have been identified by a health professional as being at high risk of type 2 diabetes, heart disease or stroke; or
18 years and over with pre-existing conditions (previous history of gestational diabetes, or have been diagnosed with high blood pressure or high cholesterol).
